We are able to treat patients who live within a 4.5 miles radius from the Practice.
To register with the Practice, come along to the surgery between 9am -12.30pm or 2pm-5.30pm with your medical card, photographic identification and proof of address (e.g. utility bill). We will photocopy your identification and utility bill, ask you to complete a short medical questionnaire about yourself and arrange a new patient appointment with one of our Practice Pharmacists. This appointment will take place approximately 2-3 weeks from date of registration.
In order to register with a doctor and obtain a medical card for the first time you need to fill in an application form. This form is called an HSCR-1. It is available English and a range of other languages. Please go to the Business Services Organisation website, Family Practitioner Services to download this form.
